The game diverges from the traditional historical setting of World War II shooters. It presents an alternate history where the Nazi regime won the war. The story begins in 1946, where the protagonist William "B.J." Blazkowicz suffers a severe head injury during a raid on a Nazi fortress, leaving him in a vegetative state. He awakens 14 years later in 1960 to find a world dominated by advanced Nazi technology, including giant mechs and lunar bases. The player must join a resistance movement to overthrow the regime.
